Upon hiring, you will provide the Staffing Team and the Case Managers with your Base Schedule, Skillset & Coverage Area. Patients will be assigned according to the information given during hiring. Any deviation from the base schedule needs to be pre-approved by HR with an advanced notice of minimum of two weeks. (For more details about the base schedule please refer to “Field Staff Per-Diem Base Schedule Terms and Conditions”) If not followed, this will be subject to immediate termination. 2. RN/LVN/CHHA/CP accepted cases must be completed once assigned/accepted/agreed and cannot be forfeited. It is mandatory for the employees to stay with the same patients until the patient is discharged from the Agency for the best patient care results/practices 3. In case of a personal emergency, all employees must email hr@tarlani.com about their absence. LVNs & CHHAs must communicate directly with the patients to re-schedule to the soonest available date. Those patients that are unable to be re-scheduled, must be notified that the agency will send a covering clinician. Also, include those patients in the email to HR in order to safely redirect them. For all other disciplines, must email hr@tarlani.com with a list of patients that need to be covered, and then notify the patients, that a covering clinician will be sent. 4. Missing more than three visits without proper explanation will lead to termination and be reported to the Board of Nursing of CA, as negligent conduct which jeopardizes the patients’ safety and wellbeing according to all the State laws and regulations. 5. Cancellation, missed visits without any report or reason will result in disciplinary action in accordance with the company policy with further consideration for termination. 6. Scheduled patient visits, must be communicated with Case Management or Clinical Supervisor to make any adjustments based on patient's change of status. Any non-compliance with accepted scheduled visits and independent decision making about no longer visiting patients, will be subject to reporting to the Board of Nursing of CA. 7. The Field employees must follow the patient's plan of care. It is the responsibility of the clinician to educate themselves to the patients history prior to the visit by looking into the patient's chart. 8. Employees are responsible to report daily visits even if the visit is missed. All employees must always answer phone calls from Tarlani Healthcare, if unable to answer, send a text that you will call back. 9. All field employees must confirm their visits 24 hours prior to their visit.Additionally, on the day of the visit, the field employee must call the patient before starting the drive to the patient's home; to double confirm the visit. If the patient does not pick up phone calls, don't go, instead contact the scheduling team & they will work to locate the patient. 10. All employees are expected to dress in a manner appropriate to the healthcare environment, or as directed by the patient/client family. This includes personal hygiene, jewelry, hair, and makeup. 11. Avoid engaging in personal discussion with patients, families, or caregivers. Avoid voicing personal opinions about patients, families, or caregivers. Avoid abusing patient's hospitality, use the telephone only in emergency situations or to call the office. Refrain from using patients trash cans to dispose of visit trash. 12. Do not offer medical advice. Do not transport patients, families, or caregivers in a car. 13. Do not take anyone into the patients' homes with you without administrative and patient approval. 14. Do not smoke/vape in the presence of a patient/client. 15. If you have any problem, incident, or accident on the job, do not discuss it with the patient, client, but call TARLANI HEALTHCARE immediately. 16. If you are to be relieved by someone else, do not leave until your relief person has arrived. 17. If the patient/client/facility asks you to stay longer than your assignment or to leave earlier, you must call TARLANI HEALTHCARE first, for approval. Refer to Directory List. 18. Under no circumstances are you to ask for, or accept, any money from your patient or take-home property that belongs to the patient/client. There shall not be any involvement with the patient/client/s financial affairs (i.e., check writing) 19. Always educate patients and caregivers to be independent in care. Nurses must make sure to provide teachings every visit - those must be short, comprehensive in a very accessible language - one at a time and please always validate prior instructions the next time you see the patient. 20. All patients' visit notes/care related documentation are mandatory to be submitted in Synergy at most 2 days after. If, for some reason you have missed the deadline, please contact Payroll@tarlani.com 21. If the visits and corresponding documentation is not submitted in the timely manner – it impairs our payroll and billing. The visits which documents/notes were not submitted by the deadline will automatically roll over to the next payroll. No extensions will be made for late submissions of route sheets and/or documentation. 22. The patient’s documentation is the property of the Agency, patient and upon warrant – the Government. The nurses must submit the documentation within the timeline provided by the agency. 23. Agency is required to verify employees' credentials are up to date. The employees will be receiving notifications via Synergy if the credentials are close to expiration. Once received, please be sure to send the updated credential(s) as soon as possible or make the proper appointment to update it. If the credential expires and the employee hasn’t provided the renewed copy to the agency, Tarlani will be forced to temporarily remove the employee from the field until all the requirements are cleared. The Agency does not reimburse for any credentialing cost. 24. It is your responsibility as a visiting staff to ensure you have enough basic supplies at any given point when you are ready to start your day. The list of the supplies that the nurse is required to have has been provided before and after the orientation. The DON or Clinical Observer will review the nursing bag during the orientation to confirm that it is up to Home Health Standards. 25. The basic wound care supplies and the Badge will be provided to you by the Agency. RNs will also be provided with the Admission Books. Always wear a TARLANI Healthcare badge during the visits. Licensed personnel must always carry their current nursing license and CPR while on the assignment. 26. After the initial orientation it is the nurse's responsibility to stop by the office to replenish the supplies. If you are located further than 20 miles from either of the company’s locations, please send the request for the supplies to supplies@tarlani.com no later than 1 week in advance for us to prepare it and ship it to you. 27. During the visit report any patient-specific supplies that are needed. You must send a message in Synergy as a request to case managers & their group. Keep in mind that some supplies might need to be approved by the insurance and that might take a few days. Check with Case Manager for details. 28.It is your responsibility to ensure that all diabetic patients and patients with all types of injections have a sharps container present during each visit by requesting supply@tarlani.com for the said supplies. Failure to report or not having a sharps container will be subject to disciplinary action. The Agency is not liable for any injuries due to not having a sharps container. Sharps Containers are available at Montrose & Colton office locations for pickup. 29. When one needs to purchase any supply due to unforeseen circumstances, it is required to contact and discuss the issue with the case management department. Once it is approved, one will be asked to submit the proof and receipt for reimbursement. The reimbursement will be included in your next payroll. 30. Should the Tarlani nurse decide to resign, that employee is mandated to perform all assigned patient care until the Agency is able to safely redirect the patients to a different nurse care. Minimum 2 week notice must be given to hr@tarlani.com and followed by the resigning employee. 31. The Tarlani Nurse must send weekly photographs of patients’ wounds and any other openings through Synergy messages. Make sure to use the Tarlani Ruler, and include the patient's name, date, measurement, location for each wound. Once a wound is reported as healed, it must be accompanied with a wound picture to be considered as officially healed.
